INTRODUCTION
You are being invited to participate in this research study because you are either pregnant or were pregnant in the past two years in Canada and have experiences with vaping tobacco or nicotine during your pregnancy.
PURPOSE OF THE STUDY
The goal of this project is to understand women’s experiences of, and attitudes about cannabis and nicotine vaping during pregnancy. The information collected will help to create health information for health care practitioners and women.
YOUR PARTICIPATION IS VOLUNTARY
Your participation in this project is entirely voluntary. It is up to you to decide whether or not to take part in this study. This consent form will tell you about the research project, why it is being completed, what will happen to you during the study, and the possible benefits, risks and discomforts associated with participation.
Participation in the survey is voluntary. If you decide to take part in the study, you can withdraw at any time in the future without providing reasons for your decision.

STUDY PROCEDURES
You have been invited to participate in a survey through posts on websites or social media channels such as the Society of Obstetricians and Gynecologists of Canada (SOGC), BC Women’s Hospital, or REACH BC.
The survey will include questions about vaping experiences leading up to, during pregnancy, and immediately postpartum. You will be asked about your patterns of use, reasons for vaping, context of use, and knowledge of health effects. We are also collecting data on demographic characteristics such as race/ethnicity, province you live in, marital status, etc. This will be used to understand access to vaping products across Canada and help create specific advice for certain/specific groups of women.
If you choose to participate, you will be asked to consent to complete an online survey. You consent to participate by clicking yes to the question, “Do you consent to participate?” After you consent, the survey questions will be available.
If you decide to take part in the study, you are still free to withdraw at any time in the future without providing reasons for your decision. The survey will take about 20 minutes to complete and will be available to complete until May 31, 2021.
Through participating in the survey, your identity will remain confidential, and at no time will your name or identity be disclosed or publicly stated by members of the research team.
At the end of the online survey, you can choose to participate in an online or phone interview. If you choose to participate, you will be able to leave your contact information for the research team.
POTENTIAL RISKS
There are no potential risks of being involved in this study.
POTENTIAL BENEFITS
There are no direct benefits to you for participating in the survey. However, your responses may benefit women seeking information about vaping during pregnancy in the future.
STUDY RESULTS
The information from this study will be used inform a) the creation of informational materials for pregnant and postpartum women who vape, b) guidance for providers to support conversations with women, and c) resource materials for interested healthcare practitioners, and health system planners.
A summary of the results will be shared but will not include any of your personal or identifying information. The study findings will also be written up in presentations and journal articles. This data will not include identifying information, such as your name.
CONFIDENTIALITY
Identifying information (such as name, location) will not be collected for the survey. Only the research team will have access to the information collected. Computer data and files will be stored as encrypted files on password protected computers and databases by the Project Manager. This information will be erased and destroyed when the project is complete. To manage the risk that this information may identify individual participants, the data will be kept strictly confidential and all results will be combined before reporting.
If you decide to participate in the online or phone interview, you will be invited to leave your contact information. Your identity will be kept confidential by the research team, and at no time will your name, address or any other personal identification be publicly released by the research team. Your name will not be in any publications or reports produced by the research team.
REMUNERATION
Participants who choose to provide their contact information at the end of the online survey and complete the online or phone interview will be eligible for a $100 gift card.
